Cause: A Java exception or error was signaled and could not be resolved by the Java code.
Action: Modify Java code, if this behavior is not intended.
Comes to my mind, this is probably the only exception Open Source will ever need:
OperationFailedException: The operation failed. Modify source code, if this behavior is not intended.
Just kidding …
